Please bring the following items to your appointment:

  • Current insurance card(s)
  • Copay for your appointment, if applicable
  • Photo ID
  • Insurance Referral, if required
  • Current copies of any testing or scan such as a CT, MRI or Ultrasound
  • List of current ENT related concerns
